11/10/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival the home, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her afternoon. Ms. Broadnax verbalized to QMHPA her afternoon was well. Intervention: QMHPA began the session by inquiring about Ms. Broadnaxâs current thoughts, feelings, and emotions. QMHPA helped Ms. Broadnax identify ways to implement her passion for cooking and baking. QMHPA helped Ms. Broadnax identify way she can develop a schedule to be available to do some of the things she enjoys doing through linking and referrals. QMHPA and client develops a To Do list. QMHPA assisted Ms. Broadnax with identifying her options with respect to increasing her self -esteem and self- image so that she can decrease her feelings of paranoia from daily to 2-3 times per week. QMHPA assisted Ms. Broadnax with identifying ways she can reduce conflict with other through role-playing exercises. QMHPA explored with Ms. Broadnax how to make decisions based upon immediate needs and priority. Response: Ms. Broadnax appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Ms. Broadnax acknowledges that she is often overwhelmed by others but did contribute her anger and frustration on her bad moods from not being able to relax. Ms. Broadnax stated, âSometimes I feel like my âthere is no one for me and those that should be there for me is not and they are the ones that want to use meâ. Ms. Broadnax managed to use good decision-making skills that will help her build her self -esteem and her confidence while resolving conflicts however, she did not get the connection with how the two work together Plan: QMHPA will continue to teach Ms. Broadnax how to identify as well as process triggers and educate her regarding positive coping skills and interpersonal skills 1-2 times (Weekly).
